multiform

multiform allows you to create multi-page HTML forms which pass values through to a final email gateway. This is ideal for very large HTML forms which might otherwise cause browser bugs to manifest. For instance, Netscape has trouble dealing with forms with hundreds of input fields.

This script is written in Perl and comes with installation and configuration instructions.
